PARENTAL CONSENT FORM
Visitors aged 10 to 15 may enter the club under parental supervision, or with this signed consent form and an adult chaperone.
WHEN IS IT NEEDED?
Our events are 18+ by default. At concerts, visitors over 16 may enter on their own; between 10 and 16, parental supervision — or this form plus an adult chaperone — is required. The age limit of each event is always stated in its Facebook event.
HOW IT WORKS
- 1Download and print the form.
- 2A parent or legal guardian fills it in and signs it in front of two adult witnesses.
- 3The minor arrives at the event with an adult chaperone, bringing the signed form.
When the concert or the programme open to minors ends, the minor must leave the club — making sure of this is the responsibility of the parent, legal guardian or adult chaperone.
